Registration Form (After School, B-CORE, A-CORE, and Private)

- All students will be accepted during OPEN ENROLLMENT WEEKS.
- Open Enrollment week is the week before the classes start, for all sessions.
- All Payments must be made before the First Day of Classes
- We except Credit Cards/ Debit Cards/Checks/ Cash / Cashier Checks. .
- A declined credit card/Check will be charged a $25 processing fee.
- All classes are scheduled in 4 week sessions. There will be no refunds for the classes.
- No make up classes are available
- Classes are not transferable
- Please plan to attend your classes

- All students who sign up for 8 week session will receive a 10% discount fee.
- lease wear non-marking, non back soled shoes, badminton shoes is highly recommended. Badminton shoes can be purchased at the Pro-Shop.
- Please dress appropriately, warm up pants, t-shirts and shorts (no jeans, no dress pants, no dress shirts and etc). Badminton Apparel can be purchased at the Pro-Shop.
- B-Core must enroll in 2 consecutive classes and A-Core must enroll in 4 consecutive classes.
- You can "borrow" a racket from the club if you don't have a racket to use based on first come basis. The Club cannot guarantee a racket available for use every time.
- All Parents must sign application form turned into Ralph Cervantes, before any classes can be attended.
- Requests for cancellation of Auto-Enrollment 1) B-core must be made the 6 week prior to the end of the registered session. 2) A-core must be made the 14 week prior to the end of the registered session. 3) Or Auto-Enrollment enrollment will resume. (Does not apply to Afterschool/Private lessons)

Before the Classes start.
Parents can choose the option of PBC application form! Or Auto-Enrollment, fill out the
information on our website. Your children automatically enroll in the next 8 week sessions.
This option is highly recommended because our class space is limited and this is the only
way to guarantee the students class spaces for future classes.
Requests for cancellation of Auto-Enrollment must be made 2 weeks prior to the end of the
registered 8 week session.
